I verified other items like Heartward Amulet and Breastplate of Valor were changed because when i looked at Blackthorn Hammer it does not reflect the changes in the season 9 update notes.

ok, so something i just noticed was that instead of having 300 health and the 25 physical and magical protections added it has 400 health in game, so i am guessing that it was a PTS thing and the protections were too over powered but they still wanted to buff it a bit when they removed them?

Yes it was reverted and then nerfed, it's at the bottom of patch notes

SmiteFire & Smite
