Cool idea, maybe reduced damage from his 1 if he activates early, and I think the passive might be a bit strong. Especially late game, 5% over 5 seconds to a bruiser is incredible sustain mid to late game. Perhaps increasing the duration of the heal? So its 5% over 10-15 seconds?

5% heal is also fine. Ideally you have about 3000 health at level 20. A 5% heal would only be 150 health. And it doesn't stack either. The passive is also as fine as it is. Do note that I take everything into consideration, and balance is at top priority. If something was overpowered, I would realise it.
