That passive is a bit overpowered, since it's basically a Cabrakan passive that also provides a 5% bonus to damage. Also, 10s slow? You might want to make the slow a bit more powerful, but reduce the duration. For your third ability, I think you should scale the healing up slightly each time you level it up (5% of your maximum health healing to everybody isn't going to be overpowered or anything). A bit of an excessive description in your ult (I don't think you need the "whoever doesn't die" bit), and you haven't said how long the dot lasts for.
The idea of a support-oriented warrior is pretty nice, though.
