Also you don't need mana chalice at all for Sylvanus.
atm I find your guide quite lacking on info. pretty much all the text is the welcome message right now. No notes on the build don't give a lot of info and your build kinda misses a starting buy section (what you grab from your first gold so Guardian's Blessing and whatever pots/chalices you get.
Lastly on consumables why is Chalice of Mana (and oracle) listed but not chalice of health. IMO health chalice is a better chalice to have than mana chalice. The reason why is because health is a more important resource especially for a support who will try to tank basics for the adc.
Also you will gain more regen over the course of the game making mana not as much of an issue especially since you also have the Verdant Growth mana regen.
With early mana management and some mana potions Imo you won;t even need mana chalice.
This is also why meditation was seen as a noob item during S1 (excluding assault) and even know it still stays mostly an assault relic as the heal isn;t that relevant compared to Magic Shell

As I said in another comment below, I'd get Lotus before Thebes. I also recommend Pestilence after Lotus for anti-heal instead of Toxic Blade; sure Sylvanus' basic attacks can do some damage, but you'll get more utility (defense and health) out of pest. If you really need extra anti-heal (e.g. your teammates are oblivious and don't buy any), get Divine Ruin. Remember that's a very niche situation, you'll generally want to skip it. If I remember correctly, anti-heal items don't stack, so multiple ruins, Brawler's Beat Sticks, and blades aren't recommended. I could be wrong and I'm not sure how pest plays into all of that. Still, it's an "always on" item versus the others which require you to either hit a basic or an ability and their effects are temporary, while pest's is permanent as long as an enemy is in range.
Relic Dagger is a good pickup in combination with Blink Rune, but the latter should be your 2nd relic and dagger your 6th or 7th item provided you don't need something else (it can also be your shoes replacement for extra movement speed since Syl's as slow as molasses going uphill).
Speaking of relic dagger, I forgot to talk about the relics. :/ Anyway, your first one is generally Magic Shell or Heavenly Wings depending upon the opposition: the former is great against multiple basic attackers because it gives 2 stacks of block when upgraded (it's also great against execute and burst ults like Than, Ao, Achilles, and Poseidon); the latter is for retreating from highly mobile gods (those with leaps, dashes, etc.) or chasing down said gods. There's some debate about meditation cloak, but I don't think you need it due to your sustain and the upgrade isn't very good. It can still be a niche pick, but it depends on your matchup. It's one of those where if you like it, keep it, but try out the two I mentioned first.
I see you got Rod of Asclepius which is good, because while its passive isn't what it used to be, its stats are still useful and Syl's healing is based off of his power, so that'll help out Wisps.
All-in-all, I think you have a pretty solid guide so I've given you my +1. I'd like to hear your thoughts on my suggestions though.
Personally don't feel Soul Reaver is a good item for him unless you weren't playing Conquest and were going for a bruiser build. You're probably better off just getting more protection-type items.
But the rest of the item choices look fine.
Chronos pendant doesn't really bring enough to the table for a support considering that there are better items like Oni Hunter's Garb (great for blink + ults), Spirit Robe, Witchblade, Pythagorem's Piece (helps when your wisps are down or you want to save them), and Relic Dagger.
Since you have good self heal, you can probably drop the Chalice of Healing. I can see a case for Chalice of Mana, but I think getting 3x Healing Potions and Mana Potions would be a better start. In fact, I'd start with Guardian's Blessing, Shoes, and pots. Also, Chalice of the Oracle shouldn't be bought until your first back and you should be picking up a Sentry Ward as well.
Gem of Isolation is another item I would drop completely because Verdant Growth's root last for 1.25s, which is over half the time the slow from gem applies; Nature's Grasp doesn't do any damage, so it won't apply the slow; and Wrath of Terra's knockup lasts at least 1 second, so it's also eating into gem's passive.
Get rid of the skill descriptions because they're aren't needed since your readers can hover over the skills to read them.
For build order, I would go with the start I listed above, then Shoes of Focus, lotus crown, Oni Hunter's Garb, Gauntlet of Thebes, situational like spirit robe, Midgardian Mail, etc.
Speaking of situational items, you have none. You should list Traveler's Shoes (great for rotations and MP5), Reinforced Shoes (if you're up against a lot of tick damagers like Anubis), Shoes of the Magi (if your ADC can poke early and you're winning lane), Hide of the Nemean Lion (reflect those pesky crits back at the attacker!), and the ones I mentioned above.
Use the skill builder and item list instead of listing your skill sequence. Branmuffin can walk you through how to use those if you're not sure. For the items, the left most is what you buy first, then it goes in sequence from left to right. You can list 8 items, but make sure you note that one is the blessing replacement and the other is the shoes replacement after you buy Elixir of Speed.
I'm going to hold off on voting until I see what changes you make. You have a good starting presentation, I just think it needs polish.
