PASSIVE - Your abilities heal you for 3.75% of damage dealt. Each time anything dies within 80 units you gain a stack. Gods, Large Jungle monsters and Bosses provide 5 stacks. At 100 Stacks Soul Eater Evolves, gaining 20 Physical Power, 10% Physical Ability Lifesteal.

EVOLVED SOUL EATER (Max Stacks):
+50 Physical Power
+15% Physical Lifesteal
+10 Physical Penetration
+10% Cooldown Reduction

EVOLVED PASSIVE - Your abilities heal you for 15% of the damage dealt to targets.

Does it proc on basic attack stims, like aimed strike or Butcher Blades?

Actually...I'm not sure about Aimed Strike. Aimed applies its own ability damage, and note that Hydra's Lament procs on the basic attack FOLLOWING an Aimed Strike hit. I think you might get that heal from the Aimed Strike...I'll test out and see.
